Case Manager resume example

Case management is coordination work: you assess what a client needs, build a plan, connect them to services and stay with them until the goals are met. The same title covers hospital discharge teams, health plans, housing programs and community mental health, so a program director first checks whether your setting and population match theirs. After that come caseload size and what changed for clients, told in counts and rates rather than one person's story.

How to Write a Case Manager Resume - Key Takeaways

  • Keep a newest-first, reverse-chronological layout on 1 page, or 2 once you have 10+ years, supervisory work or roles in several settings.
  • Open with a 2- to 3-line summary that names your setting, your population, your caseload size and your best client outcome.
  • Give each bullet a case management number: active clients, intakes a month, care plans reviewed, linkages completed or notes filed on time.
  • Count clients by group, never by name, initials or a detail that points to one person.
  • List a state license first under Certificates, then the CCM, ACM or C-SWCM with its issuer and the year you earned it.
  • Name the EHR and benefits systems from the posting in your skills and bullets, then test the file with an ATS resume checker.

What program directors look for in a case manager resume

A program director or clinical supervisor reads your resume for 3 things: the population you served, the size of the caseload you held, and what happened for those clients. O*NET lists interviewing clients and reviewing records to assess their needs, coordinating treatment with counselors, physicians and nurses, and recording progress toward treatment goals as core tasks. It also names the practical side, such as setting up appointments and arranging transportation so clients can follow their plans.

Case managers work in community mental health, hospitals, health plans, child welfare, housing and substance use programs, and each setting counts its work a little differently. Build the resume from these sections so a screener and an applicant tracking system both find what they need:

  • Name, a headline with your license or credential (for example, Case Manager, LSW, CCM) and contact details
  • A 2- to 3-line summary under your name, which prints without a heading
  • Work Experience, starting with the program you work in now
  • Skills, with your EHR, benefits and office software
  • Certificates, with any state license on the first line
  • Education, with your field placement for recent graduates
  • Optional extras: Languages, and one of Key Achievements and Volunteer Experience

Best format for a case manager resume

Choose the reverse-chronological resume format: your current role first, each one with its title, agency, location and MM/YYYY start and end dates. Case management careers tend to run from case aide or intake worker to case manager, then senior case manager or supervisor, and newest-first order puts your current caseload at the top. Use a combination format, with a skills section above your history, only if you are entering from another field and your closest experience sits further down the page.

The 3 resume formatsWhich format works best for a case manager resume?Use the reverse-chronological format unless one of the other two fits your situation better. Each card opens its full guide.

Stay on 1 page with under 10 years in the field. Move to 2 pages when you have supervised other case managers, run a program, or worked in several settings, such as a hospital and a community agency, and keep your 2 latest roles on page 1.

Case Manager resume format cheatsheet
SettingWhat to use
Length1 page under 10 years; 2 with supervision, program or multi-setting experience
DatesMM/YYYY on every role, and Present for the program you work in now
Job title lineYour title plus the program: Case Manager, Assertive Community Treatment
Client detailsCounts and groups only; no names, initials, ages or case stories
Photo and privacyUsually no photo in the US and Canada; leave off age, marital status and Social Security number
File typePDF, or a Word file when the agency's application portal asks for one

Most US and Canadian resumes carry no photo. Give that space to the line under your name instead, where your license and CCM tell a screener at a glance that you meet the posting's requirements.

Case manager resume summary vs. objective

If you have held a caseload in a paid role, write a resume summary that names your setting, your population, your caseload and 1 outcome. If your case management so far is a field placement or a related job, open with an objective built like these resume objective examples: the role you want, then the closest proof you have. Finish the bullets first, then lift the summary's number from one of them.

Case manager resume summary formula

Title with X years in setting serving population. Carry caseload size and your strongest client outcome, with a number. License, CCM or ACM status.

Example

Case manager with 7 years in community mental health, serving adults with serious mental illness in western Wisconsin. Carry 34 active clients and moved 19 of them into supported housing in the last 12 months. CCM since 2023.

Don't

Hardworking case manager with strong organizational skills, seeking a role where I can support clients and families.

Do

Health plan case manager with 4 years in telephonic care management, carrying 65 members with diabetes or heart failure. RN and CCM.

The rewrite gives the setting, the caseload and the credentials a health plan screens for.

How to write case manager work experience bullets with numbers

Follow the standard rules for listing work experience on a resume, with 3 to 4 bullets under your current role and 1 to 3 under older ones, each starting with a verb. Case management already runs on counts: active clients, intakes, care plans due for review, referrals made and how many of them turned into a service. Pick the counts your next supervisor tracks too.

Numbers that prove case management work
AreaWhat to countExample bullet
CaseloadActive clients, population, court-ordered or high-acuity shareCarried 28 adults on a forensic mental health caseload, 6 of them under court-ordered treatment.
Assessments and plansIntakes a month, plans written, review cycle metCompleted 15 intake assessments a month and updated each care plan inside its 90-day review window.
Linkage to servicesReferrals, completed linkages, first appointments keptLinked 41 clients to primary care in a year; 33 kept their first appointment.
Housing and benefitsApplications filed, approvals, days to placementFiled 22 SSI/SSDI applications for clients, with 16 approved on first review.
Transitions of careFollow-up after discharge, readmissions, ER visitsReached 95% of clients within 7 days of a psychiatric hospital discharge.
DocumentationNotes on time, chart audit results, billable unitsFiled 98% of progress notes within 24 hours across 3 quarterly chart audits.

Here is how those numbers sit in a community mental health entry.

Sample entry: case manager in community mental health

Work Experience

Case Manager, Community Support Program04/2022 – Present
Tessbrook Family Services | Eau Claire, WI
  • Carry 34 adults with serious mental illness, meeting each one at home or in the community at least twice a month.
  • Moved 19 clients from shelters or unstable housing into supported apartments in 12 months, working with 3 landlord partners.
  • Cut 30-day psychiatric readmissions on the caseload from 9 to 4 a year by booking a home visit within 72 hours of each discharge.
  • Kept 97% of progress notes inside the 48-hour documentation window through 4 quarterly audits.
Case Manager07/2019 – 03/2022
Marlowen Community Health | Chippewa Falls, WI
  • Completed 12 intake assessments a month for an outpatient program serving 2 counties.
  • Filed 31 Medicaid and SNAP applications for clients; 27 were approved on first review.

Confidentiality applies on the page as well. A supervisor who works under privacy rules such as HIPAA will notice a bullet that tells one client's story in enough detail to recognize them, so write about groups, counts and results.

Don't

Helped a 52-year-old veteran with PTSD get his own apartment after 2 years living in his truck.

Do

Moved 12 veterans from emergency shelter into permanent housing in 2025 through voucher referrals and landlord outreach.

The rewrite keeps the result and removes every detail that could identify a person.

Don't

Responsible for managing a caseload and coordinating services for clients.

Do

Closed 11 of 30 cases in 2025 with every care plan goal met, working across 9 partner agencies.

Case manager skills and software for your resume

Choose 6 to 8 hard skills for your resume that the posting asks for and your bullets already prove. Assessment, care planning and resource linkage fit nearly every case manager resume; utilization review, discharge planning or benefits enrollment belong only where your role included them.

Hard skills for case managers

  • Psychosocial assessment
  • Care plan development
  • Resource and referral linkage
  • Crisis intervention
  • Discharge planning
  • Medicaid and SNAP enrollment
  • Motivational interviewing
  • Trauma-informed care
  • Utilization review
  • Clinical documentation

Soft skills case management supervisors value

  • Client advocacy
  • Professional boundaries
  • De-escalation
  • Clear, timely notes
  • Cultural humility
  • Prioritizing a full caseload

O*NET lists Mental Health Case Manager among the job titles for mental health and substance abuse social workers, and flags Microsoft Outlook, Microsoft Office and Microsoft Excel as in-demand technology for that group, with client records software among its tools. Your agency's record system tells a reader more, so if the posting names Epic, Netsmart myAvatar or a local HMIS and you have charted in it, write that exact name in Skills and once more inside a bullet.

CCM, ACM and social work licenses on a case manager resume

Put certifications on your resume under Certificates, each with its issuer and the month and year earned, and list a state license on the first line. The BLS says all states require clinical social workers to be licensed, and some states also require nonclinical social workers to hold a license or credential. If you are an RN case manager, your nursing license takes that first line instead.

Case management credentials and what each one takes
CredentialIssued byWhat it takes
CCMThe Commission (Commission for Case Manager Certification until 02/2026)A health or human services license, such as RN, LCSW or LPC, or a bachelor's or graduate degree in social work, nursing or a related field with supervised field experience; 12 months of full-time case management under a CCM, 24 months without one, or 12 months supervising case managers, within the past 5 years; then the exam. Renewed every 5 years with 80 CE hours.
ACM-RN or ACM-SW (Accredited Case Manager)American Case Management Association (ACMA)An RN license, or a social work degree or license, plus at least 2,080 hours of supervised, paid case management work in a health delivery system, then the exam.
C-SWCM (Certified Social Work Case Manager)NASWA BSW from a CSWE-accredited program, 3 years and 4,500 hours of paid, supervised post-BSW experience, and a BSW-level state license or a passing ASWB BSW-level exam score.
State social work license (LSW, LMSW, LCSW and others)Your state's social work licensing boardTitles and rules vary by state. Clinical licenses typically require an MSW and supervised clinical experience.

How to write the license and CCM line

Copy the license title exactly as your board issues it and add the state, such as Licensed Social Worker (LSW), Ohio, then repeat the abbreviation in your headline. The Commission does not count internships, practicums or volunteer work toward CCM experience, so a new graduate should leave the CCM off entirely. Once you meet the experience rule and book the exam, list it under Certificates with the month it is scheduled, and add CCM to your headline only after you pass.

Sample Certificates section: CCM and NASW credential

Certificates

  • CCM, Commission for Case Manager Certification, 09/2023
  • Certified Social Work Case Manager (C-SWCM), NASW, 03/2024

How to list a BSW, MSW or nursing degree on a case manager resume

The BLS reports that a bachelor's degree in social work is the most common requirement for entry-level nonclinical social work jobs, while clinical social workers typically need an MSW. Psychology, sociology and nursing degrees also lead into case management. Follow the usual steps for listing education on a resume: degree name, the school and its city, and the month you graduated, with a GPA only if it is 3.5 or above and you finished in the last 2 to 3 years.

For new graduates, the field placement is the strongest line in Education: name the type of agency and the hours, because it is the first caseload a reader can see. Check your state's rules before you write a license plan, since a BSW alone does not lead to licensure everywhere; the University of Wisconsin-Eau Claire, for example, lists California, New York and Washington among states that require an MSW.

Sample education entry: BSW with field placement

Education

Bachelor of Social Work (BSW)05/2019
University of Wisconsin-Eau Claire | Eau Claire, WI

Field placement: 450 hours with a county adult protective services unit.

Entry-level and career-change case manager resumes

Without a paid caseload yet, an entry-level resume layout works best: a short objective, Education near the top with your placement, then any work where you tracked people and services. Count each item the way a case manager would.

  • Field placement or internship: clients assessed, referrals made, the program and the semesters, under Education or Work Experience.
  • Case aide, intake or direct support roles: residents supported, intakes screened, shifts documented.
  • Crisis line or shelter volunteering: hours a week and calls or guests a shift, listed under Volunteer Experience.
  • A second language: list it under Languages with your level, following the rules for language skills on a resume, and mention any interpreting you did for clients in a bullet.

Don't

Recent social work graduate looking for an opportunity to grow and gain experience in case management.

Do

BSW graduate seeking a case manager role in adult behavioral health. Supported 20 clients over 2 semesters in a county placement and answered a crisis line 16 hours a week.

An objective still needs proof: the placement, a client count and the hours.

Coming from youth programs, teaching, nursing, corrections or customer service, keep reverse-chronological order and open with a summary that names the case management work you already do: intakes, tracking participants, connecting families to services. On a career-change resume, the first 3 bullets under your current job should be its most case-management-like tasks, with their numbers. Keep the reason for the move for your cover letter; the social worker cover letter examples show how to explain it in a short paragraph.

Case Manager resume FAQ

Do you need a license to be a case manager?

Not always. It depends on your state and setting: the BLS notes that some states require nonclinical social workers to hold a license or credential, and all states license clinical social workers. For an RN case manager role the nursing license is the requirement, so read the posting and list what it asks for first.

Can I put CCM on my resume before I pass the exam?

No. Add CCM to your name line or headline only after you pass. Until then, list the exam under Certificates with the month it is scheduled, and only once you meet the Commission's experience rule, since internships and practicums do not count toward it.

How long should a case manager resume be?

Use 1 page if you have under 10 years in the field. Use 2 pages if you have supervised staff, run a program or worked in several settings, and keep your 2 most recent roles on the first page.

How do I describe clients on a resume without breaking confidentiality?

Use groups and counts, never individuals. Write "adults with serious mental illness" or "families in foster care reunification" with the caseload size and the outcome, and leave out names, initials, ages, diagnoses tied to one person and any story a former colleague could recognize.

How do I show remote or telephonic case management on a resume?

Add Remote or Telephonic to the job title line and keep the employer's city. Then give the numbers remote work runs on: members on your caseload, calls or outreach attempts a day, assessments completed by phone or video, and how quickly your notes were filed.

Should I list a second language on a case manager resume?

Yes, if you can use it with clients. List it under Languages with an honest level, such as Spanish (fluent), and add a bullet showing how you used it, such as the number of Spanish-speaking clients on your caseload or intakes you completed in that language.

Should I put salary expectations on a case manager resume?

No. Pay belongs in a later talk with the recruiter, or in the salary field of an online application. For a benchmark, the BLS reports that the median annual wage for mental health and substance abuse social workers was $60,280 in May 2025, and O*NET counts mental health case manager among that group's titles. Your offer depends on your setting, state and license.
